New Patient Forms
Upon entering our office, we will welcome you as a member of our family. We will request that you complete our patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.
Consultation
Next, you will have a consultation with Dr. Johnson to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Johnson to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.
Examination
After your consultation, Dr. Johnson will perform a complete chiropractic examination. This will include orthopedic and neurological tests, joint range of motion, muscle strength, posture, and specific chiropractic analyzes and diagnosis. However, nothing will be done in our office without your consent.
Clinical Imaging / Testing
Depending on your exam findings, Dr. Johnson may decide that other testing may be are necessary, such as X-rays, MRI's, Labs, etc. For X-rays and other clinical imaging, our office uses Advanced Radiology here in Columbia. For clinical tests and labs are office uses Boyce and Bynum Labs. X-rays and other tests are only recommended when deemed absolutely necessary by Dr. Johnson. If you have had recent tests or labs performed, we can either request that information upon your initial visit, or you may bring them with you if you have them on hand.
Report of Findings
Once the information is collected and examinations are performed, Dr. Johnson will give a report of findings on your first visit. These reports will answer:
- What is my diagnosis?
- What is the proposed treatment options?
- How often do I need to come in, and for how long?
- What will my treatment cost?
After reviewing your health history, goals, and examining your spine and X-rays, Dr. Johnson will discuss recommendations and notify you if your condition requires care with other providers. Dr. Johnson will provide the best treatment plan based on your specific needs and his clinical experience.
Treatment
Treatment is usually delivered on the first visit. Typically, Chiropractic Adjustments will be performed to the affected areas. You may additionally receive physiotherapy modalities like electrical muscle stimulation, and ice or heat. Dr. Johnson may also find the need to provide different muscle release techniques and manual therapy. Dr. Johnson may also find it necessary to prescribe a nutritional support plan for you, and / or to co-care your case with another health care provider. Your care will be a team approach, and your input will be valued and respected throughout your treatment regimen
